What Xtend Health Functional Capacity Assessment Covers?
Xtend Health Occupational Therapist will carry out a detailed assessment using interviews, standardized tools, and observation to evaluate the following areas:
Physical Abilities
Evaluates mobility, strength, balance, and coordination to identify areas where assistance or equipment may be needed.
Self-Care Activities
Looks at the participant’s ability to manage personal care tasks such as bathing, grooming, toileting, and eating.
Communication Skills
Assesses how the individual understands and expresses themselves, including any support required for effective communication.
Cognitive Functioning
Reviews memory, decision-making, problem-solving, and attention, with a focus on daily living and safety.
Domestic and Independent Living Skills
Assesses tasks like meal preparation, household chores, shopping, and the suitability of current living arrangements.
Social and Community Participation
Evaluates the individual’s ability to build relationships, engage in social settings, and participate in community life.
Employment and Education Identification
Reviews readiness or current engagement in work or study, identifying any support or adjustments required.
