Early Childhood Intervention
At Xtend Health, we are passionate about supporting children and families to thrive. Our dedicated team provides tailored therapies, including Physiotherapy, Occupational Therapy, Speech Pathology, and more. These services are designed to empower participants and help them achieve meaningful outcomes in their daily lives.
We work with children aged 0–9 years and their families to provide holistic, family-centred care that focuses on building skills, confidence, and independence. With services available across Melbourne, we ensure early intervention is accessible, effective, and impactful for every child.
Key Worker Model
Our Early Childhood Intervention services follow the Key Worker Model, as recommended under the NDIS Best Practice Guidelines for early childhood supports.
A Key Worker is your family’s primary point of contact and will:
- Coordinate your child’s therapy supports.
- Work closely with you to understand your child’s strengths, needs, and goals.
- Deliver therapy directly and consult with other team members (e.g., physiotherapists, speech pathologists, occupational therapists) to provide a coordinated and consistent approach.
- Support you to develop strategies that can be used in daily routines at home, childcare, and in the community.
This model provides families with a single trusted professional to simplify communication and ensure your child receives seamless, holistic support across all areas of development.
At Xtend Health, our Key Workers collaborate with a multidisciplinary team to ensure your child gets the right expertise at the right time.
